Use this form to transfer in / rollover in a 403(b) account from another investment provider into your Nationwide 403(b) account.  If the money being transferred in or rolled into the Nationwide account will be the first dollars in the Nationwide account then an FBC Nationwide Enrollment Form should be completed as well.
